What is inside

  • ✓ Eight clear sections: when and where, driver and vehicle, what happened, injuries and police, other party, witnesses, damage and photos, and action taken with sign off
  • ✓ A photo checklist so nothing is missed at the scene
  • ✓ Space for the other party's details, insurer and policy number
  • ✓ Signature lines for the driver and the supervisor

How to use it

  1. Print one form for each vehicle and keep it in the glovebox.
  2. After an incident, make sure everyone is safe first. The form can wait.
  3. Fill it in as soon as it is safe, with the facts only.
  4. Give a copy to your supervisor, and report to your insurer as soon as you can.
  5. File the form with the vehicle's records.

Why it matters

In the minutes after an accident it is easy to forget the details you will need later. A clear record made at the time helps with insurers, with your own management and with learning what went wrong. A printed form in every vehicle makes sure nothing is missed.

Need more than this?

This free download covers the basics. A paper form can be lost, and it only helps if someone files and reads it. If you want more, we can help with:

  • ✓ An incident tracker that keeps every report, photos and actions together
  • ✓ Roadside assistance and towing when a vehicle cannot be driven
  • ✓ Repairs to get the vehicle back on the road
